About SugarRight Diabetes Management Program

SugarRight is a fully funded program that provides multidisciplinary care for complex or high-risk type 2 diabetes patients, 18 years and over.

Program Information

SugarRight is a fully funded program that provides multidisciplinary care for complex or high-risk type 2 diabetes patients, 18 years and over.

 Care is delivered by advanced-skill general practitioners in a general practice setting, under the supervision of an endocrinologist and with the support of a credentialed diabetes nurse educator, dietitian, exercise physiologist, optometrist, podiatrist, psychologist and social worker.

 The multidisciplinary team will work on a management plan addressing sugar level control, blood pressure, lipids (Cholesterol), lifestyle, diabetes complication management and patient priorities, once these are addressed and the patient is stable or the funding period has expired, they will be discharged back to the referring GP for continuing diabetes care. Referral from the patient’s GP is a prerequisite for program entry.

Education Sessions are also a part of the program. Patients can attend a 1 hour per week session delivered by our multidisciplinary team. The education sessions will run over 8 weeks and will address physical activity by our Exercise Physiologist, who will tailor an exercise program for the individual patient whether it be pool, walking track, gym or a combination.

Margaret has a Bachelor of Nursing, Masters in Clinical Nursing and a certificate of Management and Education in Diabetes. In 2017 became a Diabetes Educator and then progressed to be a Credentialed Diabetes Educator June 2021. Margaret’s services include the following;

  • Type 2 Diabetes
  • Gestational diabetes
  • Pre-diabetes
  • Diabetes self-management education
