Please contact Reception on (07) 4150 1800 to book your initial consultation with our team, and we will advise what forms will be required.
At your initial consultation please provide the following if you have them:
- Medicare Card
- Concession card such as a Health Care Card or Pension Card.
- DVA card
- Enduring Power of Attorney documents
- Previous medical history if you have access to them (if you do not have access to your previous medical history, we can request them on your behalf, just ask us how!)
- Workcover policy numbers or related paperwork if required for your appointment.
Please advise reception if translation or support services are required.
If you are bringing a service animal into the clinic, please be advised you may be asked to provide a certificate of authenticity.
Our building is fully wheelchair accessible with disabled parking at the front and rear of the building.
Please be advised, for security reasons, we do not accept or send emails regarding patient medical or clinical matters including medical records, script and report requests, or requests for appointments.
Electronic transfers can be securely sent to us via Medical Objects or Argus.

If you present with flu like symptoms (cough, sore throat, runny nose, fever, shortness of breath) the day of your appointment, or if you test positive for COVID-19, or if you have been advised to isolate the day of your appointment, or if you have come in recent contact with a positive COVID-19 case, please contact reception on 07 4150 1800 prior to your appointment.
